A New York-based healthcare provider is seeking a licensed Nurse Practitioner (NP) or Physician Assistant (PA) for a role focused on providing community-based long-term care services in several counties. About the Opportunity: Schedule: Weekdays and weekends Hours: Minimum 9 hours/week Setting: Community-based Responsibilities: Reviewing and recommending on consumer assessments Maintaining knowledge of regulatory changes and clinical quality Validating assessment outcomes using clinical skills Collaborating on quality assurance and improvement initiatives Supporting program goals through education and communication Qualifications: 2+ years of experience as an APP Master’s Degree (NP or PA Program) Active NYS Nurse Practitioner (NP) and/or Physician Assistant (PA) license Active Medicaid Number BLS Certification Computer literacy and tech navigation Experience with telehealth assessments Strong written and oral communication Interpersonal skills and adaptability Desired Qualifications: Experience with Adult/Geriatric populations Knowledge of Long-Term Care programs
