Mission City Community Network, Inc. (MCCN) is a non-profit Federally Qualified Health Center (FQHC) providing comprehensive, high-quality, and culturally competent health care to underserved communities across the San Fernando Valley, Antelope Valley, and greater Los Angeles areas. Our multidisciplinary services include Adult Medicine, Pediatrics, Women’s Health, Dentistry, Behavioral Health, and Health Education — all designed to improve the health and well-being of the families we serve. We are seeking a compassionate and mission-driven Physician (MD/DO) - Family / Internal Medicine to join our dedicated team. The ideal candidate is committed to providing equitable, evidence-based care to a culturally diverse, low-income patient population in an outpatient primary care setting. Location: North Hills, CA (San Fernando Valley Region) Location: Inglewood and Koreatown-Wilshire (SB/Los Angeles Region) Location: Pomona, La Puente, Corona, Victorville/Barstow (SGV/High Desert Region) Status: Full-Time or Part-Time Clinic Schedule Monday–Friday, 8:00 AM – 5:00 PM Saturday, 7:30 AM – 4:00 PM (availability for 1–2 Saturdays/month required) RESPONSIBILITIES, includes but not limited to: Provide primary care treatment in accordance to standard medical practice protocols Record medical history and provide physical examinations and follow-up care Provide individual patient instruction in accordance to prescribe treatment plan Prescribe appropriate medication to patients as needed according to medical protocols and standard medical practice Identify physical, nutritional and/or lifestyle problems and refer to appropriate on-site and off- site resources Identify psychological and emotional problems/issues and refer to on-site or off-site resources Consult with Medical Director or specialty physicians for patient evaluations as needed In conjunction with Medical Director, update Medical Protocols on an annual basis Adhere to Quality Improvement guidelines and participate in Quality Improvement programs for chronic diseases Supervise Mid-level practitioners as assigned Work closely with Nutritionists/Health Educators in order to provide appropriate education to the patients Participate in Provider meetings and QI Meetings as required Serve as a preceptor instructor to Nurse Practitioner, Physician Assistant, or medical students assigned Attend conferences and seminars when requested and comply with continuing education requirements Provide trainings to Nurse Practitioners, Physician Assistants and other clinic staff, on periodic basis Qualifications MD or DO from an accredited medical school. Board Certified or Board Eligible in Family Medicine/Internal Medicine. Current California Medical License and DEA registration. Current BLS certification. Minimum one year of outpatient/clinic experience preferred. Bilingual (English/Spanish) preferred but not required. Flexibility to work across MCCN clinic locations in the San Fernando Valley, South Bay/Los Angeles or San Gabriel Valley/High Desert Knowledge, Skills, and Abilities Strong foundation in outpatient family medicine and chronic disease management. Knowledge of FQHC operations and community health practices. Excellent communication and interpersonal skills with a patient-centered approach. Ability to remain calm, objective, and effective in fast-paced or emergency situations. Strong organizational and documentation skills.
