Nazareth Hospital is looking for a Family Medicine Physician to join their team in Philadelphia, Pennsylvania. Trinity Health Mid-Atlantic is a diverse integrated system providing health services including five acute care hospitals, three home health organizations, wellness centers, physician practices, and three federal PACE programs. Job Description Full-time position within 2701 Holmes Avenue Physicians Office Building at Nazareth Hospital 1:7 call, including some weekends Provide direct services through comprehensive evaluation, diagnosis, treatment planning and treatment Provide procedural services within the scope of specialty training and clinical competence Communicate appropriately, following confidentiality guidelines, with internal and external providers to ensure quality and continuity of care Participate in the education of health professions students assigned to site rotations Medical record documentation will be in compliance with policies and procedures. Maintain an active Pennsylvania physician license, board certification in family medicine specialty, and controlled substance registration with the Drug Enforcement Administration. Abide by the credentialing and re-credentialing policy. Maintain participation in Medicare, Medicaid, and other third party payer programs as required by administration. Management of chronic diseases and commonly encountered outpatient acute medical conditions. Knowledge and competencies in understanding and collaborating with the region's behavioral health and social service system. Comfort providing above services to individuals with severe/persistent mental illness, substance use disorders, and housing instability.
