HarmonyCares seeks a Physician POD Leader to provide home-based primary and palliative care for medically complex, home-limited patients. You will lead an interdisciplinary POD team, oversee clinical quality, and ensure patient-centered care across in-home visits and telehealth. Responsibilities include managing panel performance, supervising clinicians, coordinating care transitions, and reducing avoidable hospitalizations. You will champion a compassionate, mission-driven culture while mentoring staff, engaging with families and community partners, and supporting continuous improvement in outcomes and access for vulnerable patients.
