HarmonyCares seeks a Physician POD Leader to oversee an interdisciplinary home-based primary care and palliative care team serving medically complex, home-limited patients. You will provide direct clinical care, lead POD physicians and advanced practitioners, optimize care pathways, and ensure quality, safety, and regulatory compliance. The Physician POD Leader collaborates with nursing, social work, and care management to reduce avoidable hospitalizations, support telehealth integration, and drive performance improvement aligned with our patient-first, mission-driven culture.
