HarmonyCares seeks a Physician Pod Leader to oversee home-based primary and palliative care for medically complex, home-limited patients. In this leadership role, you will guide an interdisciplinary pod of clinicians, ensure high-quality in-home and telehealth visits, optimize care plans, and reduce avoidable hospitalizations. You will champion a patient-first, compassionate culture, mentor providers, monitor clinical outcomes, and drive care coordination and workflow efficiency.
