Physician POD Leader

HarmonyCares seeks a Physician POD Leader to guide a home-based primary and palliative care team serving medically complex, home-limited patients. In this leadership role, you will oversee a panel of patients, provide direct clinical care in the home and via telehealth, and coordinate interdisciplinary team members to optimize quality, safety, and patient experience. Responsibilities include rounding oversight, care planning, performance improvement, and mentoring providers. You will help reduce avoidable hospitalizations, ensure evidence-based practice, and foster a compassionate, patient-first culture aligned with HarmonyCares’ mission.
