Job Description
HarmonyCares seeks a Primary Care Physician to deliver home-based, patient-centered care to medically complex, home-limited adults. As part of an interdisciplinary team, you will perform in-home assessments, manage chronic and acute conditions, coordinate palliative and transitional care, and leverage telehealth to reduce avoidable hospitalizations. You will collaborate closely with nurses, care managers, and social workers, contribute to care plans, and communicate with families and community partners, all while advancing our mission to improve access and outcomes for vulnerable patients.
