We are seeking a dedicated, reliable and compassionate Nurse Practitioner to join our primary care clinic team. The Nurse Practitioner will work closely with other healthcare professionals to deliver high-quality, patient-centered care to individuals 18+. The ideal candidate will have excellent clinical skills, strong communication abilities, and a passion for promoting health and wellness within the community. Responsibilities: Conduct comprehensive health assessments, including medical history reviews and physical examinations. Diagnose and treat common acute illnesses, injuries, and chronic conditions under established clinical protocols and guidelines. Develop and implement individualized care plans for patients, focusing on preventive care, health maintenance, and disease management. Order and interpret diagnostic tests, such as laboratory tests, imaging studies, and electrocardiograms. Prescribe medications and treatments as appropriate, considering patient preferences, medical history, and clinical guidelines. Provide patient education on health promotion, disease prevention, and self-management strategies. Collaborate with interdisciplinary healthcare team members, including physicians, nurses, social workers, and pharmacists, to coordinate patient care and optimize outcomes. Maintain accurate and up-to-date electronic health records, documenting patient encounters, assessments, interventions, and treatment plans. Participate in quality improvement initiatives, clinical audits, and peer review activities to enhance the delivery of care and ensure compliance with regulatory standards. Stay informed about current developments in primary care practice, evidence-based guidelines, and healthcare technology advancements through ongoing professional development and continuing education. Perform DOT exam (willing to train) Perform women's health assessment, PAP, Birth control, STD testing etc. Qualifications: Master's degree or higher in Nursing from an accredited program. Current licensure as a Nurse Practitioner in the state of practice. Certification as a Nurse Practitioner by a nationally recognized credentialing organization (e.g., AANP, ANCC). Minimum of 1 year of clinical experience in primary care or a related specialty. Proficiency in conducting physical assessments, diagnosing medical conditions, and managing acute and chronic illnesses. Strong interpersonal skills and the ability to communicate effectively with patients, families, and interdisciplinary team members. Demonstrated commitment to providing patient-centered care and promoting health equity. Experience with electronic health record systems and medical documentation practices. Ability to work collaboratively in a fast-paced healthcare environment while maintaining attention to detail and prioritizing patient safety. Dedication to professional growth and continuous learning in the field of primary care nursing. Preferred Qualifications: Experience working in a primary care clinic or community health center setting. Bilingual proficiency in English and Spanish is preferred but not necessary Certification in specialty areas such as family practice, adult gerontology, pediatrics, or women's health. Prior experience in managing chronic conditions such as diabetes, hypertension, asthma, or COPD. Knowledge of population health management principles and value-based care models. Working Conditions: This position typically operates in a clinical office setting within regular business hours. The Nurse Practitioner may be exposed to infectious diseases and must adhere to standard precautions and infection control protocols. HOURS: FULL Time: Monday to Thursday (9a-4pm) & Fri (830a-2pm).
