Sun River Health is looking to hire an accomplished Full-time Primary Care Physician (FM) to join our team in Wyandanch, NY. The Primary Care Physician is responsible for managing a diverse patient panel with multiple co-morbidities in a busy Federally Qualified Health Center. Care is delivered in collaboration with advance practice providers, on-site support staff, with an emphasis on preventative health and quality patient care. Diagnose and provide treatment plans for patients. Collaborate and supervise advanced provider clinicians. Complete and participate in chart review process. Referral and follow-up of patients when needed. Collaborate with other members of the Health Center in order to maintain high quality standards and achieve client satisfaction.
