Primary Care Physician

Seeking a Primary Care Physician for a senior-focused PACE program serving individuals 55+ with chronic illnesses or disabilities. Responsibilities include delivering comprehensive primary care, developing individualized care plans with an interdisciplinary team, managing acute and chronic conditions, providing preventive services, and conducting home or facility visits. The role involves effective communication, staff support, and participation in quality improvement efforts. Requirements include MD/DO with interest in geriatrics, board certification or eligibility, state license, DEA registration, and at least one year of experience working with elderly or frail populations.
