Primary Care Physician

Seeking a Primary Care Physician for a senior-focused PACE program serving individuals 55+ with chronic illnesses or disabilities. Responsibilities include delivering comprehensive primary care, developing individualized care plans, managing chronic and acute conditions, providing preventive services, and coordinating with an interdisciplinary team. Requirements: MD/DO with interest in geriatrics, board certified or eligible, valid license, DEA registration, and at least one year of experience working with the elderly.
