Primary Care Provider (Physician or APP) Primary Care Provider - Physician (MD/DO), Nurse Practitioner (NP) or Physician Assistant (PA) Department: Primary Care Location: Muncie, Indiana Reports To: Chief Clinical Officer / VP of Operations Sign-On Bonus: $10,000 for Physicians (MD/DO) Up to $5,000 for Nurse Practitioners or Physician Assistants with 3+ years of experience Position Overview Open Door Health Services is seeking a dedicated and compassionate Primary Care Provider (Physician, Nurse Practitioner or Physician Assistant) to deliver high-quality, patient-centered care. This role involves managing acute and chronic medical conditions while collaborating with an interdisciplinary healthcare team to support whole-person care. As a Federally Qualified Health Center (FQHC), Open Door Health Services provides comprehensive primary, urgent, preventive, and select specialty services, along with social support programs for individuals and families—regardless of their ability to pay. Our patient population reflects diverse backgrounds, life experiences, and healthcare needs. Location This position is located in Muncie, a mid-sized city in East Central Indiana. Recognized as one of the most affordable college towns in the United States, Muncie boasts top-tier arts and culture, entertainment, recreation, and education for all ages. Muncie is home to Ball State University, offering highly respected academic programs and a vibrant arts, culture, and community life. Open Door's flagship health center is in downtown Muncie, the city's revitalized hub for local shops, eateries, live performances, and cultural events. Living options include historic neighborhoods, new builds, downtown living, rural spaces, and everything in between. Roles and Responsibilities Commitment to ethical, inclusive, and patient-centered care. Experience working with adults. Understanding of the social determinants of health and their influence on patient outcomes. Strong clinical assessment and diagnostic skills. Knowledge of clinical documentation and diagnostic standards (ICD). Familiarity with HIPAA regulations and ethical practice standards. Excellent communication, interpersonal, and organizational skills. Ability to work independently while thriving in a team-based, integrated care environment. Comfort using electronic health records (EHRs) and digital assessment tools. Qualifications For Physicians (MD/DO) Unrestricted license to practice medicine in the State of Indiana. Federal DEA certification and Indiana CSR. Board Certified or Board Eligible. Medical degree from an accredited medical school. Eligibility for unrestricted privileges at Ball Memorial Hospital. Strong community health / public health orientation. For Nurse Practitioners (NP) Active Indiana Registered Nurse (RN) license. Indiana Nurse Practitioner authorization. Federal DEA certification and Indiana CSR. Graduate of an accredited school of nursing (BSN required). Completion of an accredited Nurse Practitioner program with certification. Community health / public health experience preferred. For Physician Assistants (PA) Possession of Indiana State Physician Assistant License. National Commission on Certification of Physician Assistant (NCCPA). Federal DEA certification and Indiana CSR. Graduate of an accredited PA program required. Successful completion of an approved Physician Assistant program, testing, and certification. Community health / public health experience preferred.
