The Physical Therapist:A.Assists the physician in evaluating the level of function, disability, injury, or disease process of individual patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests.B.Interprets plans, establishes, and exercises proper treatment program in accordance with the physician's referral, periodically reviewing the patient's program and response and making recommendations to the physician relative to the need of continuing or discontinuing the physical therapy treatment.C.Contacts the physician directly for patient reports and care as needed.D.Submits to the physician and the Agency written summary of the patient's progress at the designated intervals of every sixty (60) days and at discharge of the patient using the appropriate and designated forms.E.Participates in case conference as required.F.Works in collaboration with the physicians and other members of the health professional team to determine the needs of the patients and develop their total plans of care.G.Instructs, teaches and supervises patients, their families and other health ream personnel regarding physical therapy procedures as related to the patent's therapy programs.H.Evaluates the home environment and makes appropriate recommendations.I.Documents each visit made to the patient and incorporates notes into the clinical record at least weekly.J.Participates in staff development activities, which includes providing in-service training to other home health personnel; participates in in-service education programs monthly.K.Treat and reassess patients seen by PTA at least every 30 days.L.Conduct face-to-face patient care conference with each PTA to review progress and modification of treatment program for all patients seen by PTA.M.Performs other ordered job-related duties as assigned.Job Types:Part-time, PRN Pay:$80.00 - $150.00 per hour Benefits:Flexible schedule Referral program Medical Specialty:Home Health Orthopedics Physical & Rehabilitation Medicine Experience:Home health:1 year (Preferred) Ability to Commute:Dallas, TX 75206 (Required) Work Location:In person.
