Overview Manage the day-to-day aspects of this program, including managing the program timeline and goals outlined, supervising the Patient Navigators (Peer Counselors) and ensuring accurate record keeping, data tracking, and proper follow-up on all test positives received. Check-in regularly with the Patient Navigators to ensure they are completing their assigned duties, and to monitor the linkage to care activities. Work with the data analyst on dissemination and poster/publication submissions. Responsibilities Establish and maintain positive relationships with patients, visitors, and other employees. Contribution and participation includes data collection, analysis, implementation of and compliance with risk management and claims activities, support of and participation in Continuous Quality Improvement (CQI) teams, consistent adherence to the specific rules and regulations of the BronxCare Health System's (a) Safety and Security Policies, (b) Risk Management: Incident and Occurrence Reporting, (c) Infection Control Policies and Procedures and (d) Patient and Customer Service. Coordinate and manage all aspects of the Gilead project including management of the project timeline, project goals, coordinating case management meetings, liaising with Hospital clinical leadership.
