About Us EverCare Health Services is a Maryland DDA-approved provider committed to empowering individuals with developmental disabilities through compassionate, person-centered care. We provide services across Maryland including Personal Support, Community Development Services, Supported Living, Respite, Residential Services, Employment Services, Nursing, and Transportation Services. We are currently seeking a dedicated and organized Program Coordinator to join our growing team. Position Summary The Program Coordinator is responsible for overseeing the coordination and delivery of services for individuals receiving supports through the Maryland Developmental Disabilities Administration (DDA). The ideal candidate will ensure quality service delivery, compliance with DDA regulations, staff oversight, and person-centered support implementation. This role requires strong communication, leadership, problem-solving, and documentation skills. Responsibilities Coordinate and oversee services for individuals receiving DDA supports Ensure implementation of Person-Centered Plans (PCPs) and individual goals Monitor staff performance and provide ongoing supervision and support Conduct regular site visits, monitoring meetings, and quality assurance checks Maintain accurate documentation and ensure compliance with DDA regulations and agency policies Collaborate with Coordinators of Community Services (CCS), families, guardians, nurses, and interdisciplinary teams Assist with staff scheduling, onboarding, training, and incident follow-up Monitor individual health, safety, behavioral, and support needs Ensure timely completion of reports, progress notes, and required documentation Participate in care planning meetings and assist with service authorizations and transitions Respond professionally to emergencies, concerns, and critical incidents Qualifications Bachelor’s degree in Human Services, Social Work, Healthcare Management, or related field required Minimum of 3 years of experience in a similar role, preferably in a DDA or healthcare setting Strong knowledge of Maryland DDA waivers, HRST, PCPs, LTSS, and service coordination processes Excellent organizational and time-management skills Strong interpersonal and communication skills Ability to work collaboratively with diverse teams, families, and providers Proficient in documentation systems and service delivery software Knowledge of state and federal compliance requirements HRST Certification or related assessment experience preferred Current CPR and First Aid certification required Valid Maryland driver’s license with a good driving record required Proof of automobile liability insurance required Must possess all required credentials, licenses, and certifications Current health clearance/physical within the last 12 months required Must pass criminal background investigation and required screening checks Must be authorized to work in the United States (I-9 documentation required) Social Security card or acceptable identification documentation required TB test or chest X-ray within the last 12 months required Hepatitis B vaccination series or signed declination required Minimum of two professional references in good standing required Verification of previous employment history required Strong verbal and written English communication skills required Pay: $50,003.44 - $60,219.20 per year Work Location: In person
