Promotora JOB SUMMARY: The Community Health Worker delivers the program model and will plan and implement culturally sensitive and effective in-home family education services, as directed by service recipients, applicable stakeholders and collaborative partners, while ensuring that services provided are in alignment with the Social Determinants of Health through a trauma informed care lens as defined by the agency. ESSENTIAL PERFORMANCE REQUIREMENTS AND FUNCTIONS: Required Competencies/Skills: Healthcare and Human Services Environments: Ability to explain issues and advancements in the healthcare and human services industries. Completion of Community Health Worker certificate preferred, or experience in community-based environment. Complete an approved Nurturing Curricula and be able to utilize various learning techniques in the successful delivery of the Nurturing Curricula. At least two (2) years’ experience in the delivery of service those are resident-driven and work with families to address multiple issues in-home and/or community-based environment preferred. Maintain certification in at least one approved educational curricula required by agency stakeholder. Fluent in English and Spanish (read, write and speak) desirable. Certificate or license, as applicable, by the appropriate State agency as applicable. State of Texas vehicle operator's license and proof of liability insurance.
