Essential Duties and Responsibilities Provides prudent psychiatric and medical care, ensuring effective medication management and high client satisfaction. Consults with multidisciplinary staff and community providers as needed. Generates referrals for acute, chronic, and preventive care, maintaining strong relationships with community agencies. Educates patients on preventive care and chronic disease management, directing them to appropriate resources. Conducts follow-up care and retrieves necessary lab and imaging reports. Submits encounters for accurate service authorization and coding. Meets productivity expectations and documents in compliance with guidelines. Attends departmental meetings and contributes to grant applications and policy development. Maintains good relationships with clinic staff and ensures regulatory compliance. Demonstrates professionalism, accountability, and a strong work ethic. Participates in peer reviews and performs other related duties as required.
