Job Description
St. Coletta's of Illinois is seeking a Qualified Intellectual Disabilities Professional (QIDP) to join the Social Services Team. This individual will be responsible for the development of comprehensive Implementation Strategies (IS), coordination of services, overseeing the implementation of ISPs, and providing the general case management to ensure the individuals with intellectual and physical disabilities in services attain personal goals. Shift : 1st Pay rate : $46,000 - $50,000 (depending on certification/experience level) Responsibilities: Develops, monitors and documents person centered Implementation strategies (IS). Records information discussed at CST meetings. Explains all rights to which an individual is entitled as required by DHS. Reviews service plans monthly, documents service implementation, progress, and needs for modification. Provides a copy of a written service plan to the individual and an electronic copy to each member of the individual’s community support team. Ensures all areas of responsibility are in compliance with CARF, DMH/DD, etc., standards and/or regulations. Advocates for the consumer. Advocates for services and supports to help the consumer achieve their outcomes. Promotes and teaches self-advocacy. Monitors the health, safety and well-being of each consumer. Assures consumer’s participation in all decisions affecting his/her life. Serves as primary contact person for guardians and all service providers. Qualifications: Must be 21 years of age. Must possess a valid Illinois Driver’s License. Must be a DHS certified QIDP or willing to become certified. Bachelor’s Degree in Human Services field, including, but not limited to, sociology
