Job Description
Position Summary This position will serve as an integral member of the Facility Team and will assist with the day-to-day activities of the facility. Responsible for providing clerical and technical support in the internal monitoring system. Ensures policies and procedures are in compliance with Federal, State, and local standards. Responsibilities Coordinates and tracks the Internal Monitoring System as established in Policies and Procedures. Ensures all internal monitoring is completed, recorded and submitted to all appropriate parties including but not limited to Administrator, Director of Operations, Chief Operating Officer and others as needed. Create and Maintain a monthly tracking system for all internal and external monitoring, including visits, items reviewed, findings, and corrective actions needed. Based on internal monitoring deficiencies, assign corrective action and deadlines for those actions. Track and notify supervisors of any deficiencies lasting longer than ninety (90) days. Monitor and maintain verification that all internal and external corrective actions were completed. Collect, organize, and electronically file all internal and external monitoring forms, reports and corrective action reports. Coordinate Quality Assurance meetings and maintain minutes. Complete records reviews and chart audits for all departments at facility at least twice a year or more often as scheduled and assigned by supervisors. Completes at least weekly data checks to ensure completion of all documentation regarding programming, health and safety, comprehensive assessments, including daily observation, log sheets, and daily reports. Reviews documentation to ensure results of the Interdisciplinary Team decisions are implemented in a timely and appropriate fashion. Reviews documentation to ensure participation and active treatment is provided at natural times in multiple settings throughout the day to promote independence, reinforce skills and master behaviors. Ensures staff performance is reviewed identifying both strengths and areas of concerns using multiple tools including reviews and appropriate disciplinary action in accordance with policy. Train staff on multiple in-services, program implementation and other subjects using demonstrations, practice, observations, and feedback methods to ensure staff mastery of technique is achieved. Actively monitors programs and documentation of programs for service delivery and quality. Assist with various committee duties and processes such as coordinating committee, chairing committee, taking minutes, etc. and various other meetings as needed. Assist with staff activities as needed. Participates in Administrator on Duty, unannounced off hour visits, and other duties as assigned. Completes internal investigations as needed. Attends and participates in interdisciplinary team meetings, in-services, and competency-based trainings. Understands and adheres to all policies and procedures. Reports instances of abuse in accordance with specified guidelines in the abuse policy. Other duties as assigned. Education and Experience Bachelor’s Degree in Human Services field preferred. High School Diploma or GED required. Minimum of three (3) years’ experience in quality assurance programs preferred. Minimum of one (1) year of previous supervisory experience preferred. Strong MS Office Suite Knowledge. Required Skills and Abilities Ability to comprehend and demonstrate implementation of a variety of programs for individuals. Ability to act as a role model by maintaining and demonstrating professionalism, and a positive image. Must have compassion, patience, and ability to nurture. Must be able to read and write American English proficiently. Must maintain training competencies as assigned. Ability to understand, implement and monitor various software systems for training, record keeping and employee management. Ability to prioritize work for timely completion in a fast-paced environment with minimal supervision. Knowledge of Center for Medicaid Services rules and regulations and applicable laws and regulations. Possession of a valid Driver's License with required endorsement, along with an acceptable driving record for the past three years is required to transport.
