Quality & Incident Investigator Job Type: Temp-to-Hire Salary: $50,000-$60,000 annually (upon permanent placement) Location: Yonkers, NY | On-site (Monday-Friday, Full-time) Job Overview Madison Approach Staffing is recruiting for a Quality & Incident Investigator for our client, a human services organization. This Compliance & Risk Management role is responsible for ongoing assessment of program performance, incident investigations, and monitoring compliance with contractual agreements and regulatory requirements including OCFS (Office of Children and Family Services) and Justice Center standards. The ideal candidate has human services compliance experience, strong investigative and analytical skills, and ability to work cross-functionally with interdisciplinary teams. This is a temp-to-hire opportunity with transition to permanent employment and comprehensive benefits package. Key Responsibilities Compliance & Quality Assurance Perform reviews of program compliance with health hazard and safety risk standards Conduct billing compliance and quality performance reviews Gather accurate data on programs' compliance with applicable standards of care Monitor adherence to OCFS, NYSED, and Justice Center requirements Assist with agency accreditation renewal processes Incident Investigation & Risk Management Lead and complete internal incident investigations with thoroughness and objectivity Generate comprehensive written investigation reports with sound recommendations Provide data analytics to identify and inform on risk factors Track incidents and identify trends to prevent recurrence Ensure timely completion of health records management processes Meetings & Leadership Run meetings related to Continuous Quality Improvement (CQI), Incident Management, and Compliance Collaborate with administrators, program supervisors, and direct line staff throughout agency Respond to requests from leadership in timely and professional manner Support overall strategic plan of organization Present findings and recommendations to interdisciplinary teams Required Skills & Experience Education: Associate degree in Social Science or related field required; Bachelor's degree preferred Regulatory Experience: OCFS (Office of Children and Family Services) or NYSED experience required Compliance Knowledge: Knowledge of compliance and incident management in human services field Data Analysis: Ability to collect accurate data and conduct thorough analysis Writing Skills: Strong writing skills in English with ability to produce comprehensive reports Reading Comprehension: High reading comprehension for regulatory documents and standards Technology: Proficient computer skills including Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) EHR Systems: Adeptness to learn new software and electronic health records systems Communication: Effective verbal and written communication skills Mobile Technology: Ability to access agency electronic systems including smartphone apps (ADP) for work purposes Preferred Skills & Experience Justice Center investigation experience Experience with accreditation processes (COA, Joint Commission, or similar) Project management skills Training or background in quality improvement methodologies Experience presenting to leadership and external stakeholders Knowledge of human services billing and documentation requirements Work Schedule & Benefits Full-time position: Monday-Friday (standard business hours) On-site in Yonkers, NY Temp-to-hire opportunity with transition to permanent employment Benefits Upon Permanent Placement Generous PTO: 4 weeks vacation, floating holidays, holidays, personal and sick time Health, Dental, and Vision Insurance 401(k) with employer contributions Tuition reimbursement Flex Spending Accounts Employer-paid Short-Term Disability, Long-Term Disability, and Life Insurance Professional development opportunities Physical Requirements Ability to sit, stand, walk, kneel, reach, talk, and hear Use of close and distance vision Ability to climb stairs and carry up to 30 lbs Work environment involves no physical risk or hazardous conditions To Apply: Please submit your resume to Madison Approach Staffing.
