Referral Coordinator Community Outreach

Job Description – Referral Coordinator / Community Outreach (part-time). This position requires excellent communication skills working for a non-medical home health care agency. The objective is to generate geographically appropriate, referral-driven private-pay cases that support block scheduling and caregiver retention. Industry experience is preferred but not required The core responsibilities include: Developing relationships with hospital discharge planners Skilled nursing/rehab facilities Elder law attorneys Geriatric care managers Financial planners and wealth managers Hours & Structure 10-15 hours/week Flexible schedule Hourly pay +bonus tied to hours of care started
