Job Description
Provides support and technical assistance involving referrals for eligible beneficiaries to verify eligibility for Purchase Referral Care (PRC); enrolls patient in alternate resources benefits for which they may be eligible; contacts state agencies and sets up appointments to determine medical eligibility; coordination and management of referrals; works closely healthcare providers and members of the care team to communicate referral updates and appointment information to patients, acts as a liaison for both vendors and recipients of PRC. ESSENTIAL FUNCTIONS Reviews alternate resources prior to expenditures of PRC and alternative funds. Provides technical assistance to verify patients enrollment for all alternate resources benefits for which they may be eligible. Provides acts as a an advocate for patients in the utilization of alternate resources... Tracks, schedules and coordinates referrals to specialties.- add on other responsibilities Meets with patients to explain technical provisions of the PRC Program and third party billing procedures;. Coordinates with Patient Registration in screening for potential alternate resource eligibility, sets appointments; refers patients to Social Services and other disciplines. Compiles data on patients at other facilities (non-SCAHC) and updates information on the registration system specific to medical status (diagnosis, prognosis, and cost of health care, discharge status). Maintains correspondence and reports, answers telephones and personnel inquiries. Compile and maintain a schedule log for appointments made Assist in coordinating and scheduling transportation. Performs other duties as assigned. 6 months of related experience.
