Job Description
Job Purpose The Regional Nurse is responsible for delivering high-quality, compassionate nursing care to patients in their homes, as referred by partner organizations or internal programs. This nurse will conduct routine visits to perform wound care, administer medications, and complete general health assessments as requested by the organization. The Regional Nurse may also be assigned to perform nursing duties as assigned in a Diagnostic & Treatment Center or Medical Practice. The role requires strong clinical skills and judgment, the ability to work independently, and a patient-centered approach to care. Job Responsibilities Perform routine home visits to deliver clinical services, including:Wound care management and dressing changesMedication administration and reconciliationGeneral health and functional assessments as directed Provide nursing care in accordance with the individualized care plan, physician orders, and best nursing practice standards. Assess patient conditions and monitor changes in health status, promptly communicating updates to the care team and physician as needed. Collaborate with partner organizations, care coordinators, and internal departments to meet patient needs. Educate patients and caregivers on disease management, medication adherence, wound care, and safety in the home environment. Ensure timely and accurate documentation using the organization's electronic health record system. Adhere to all clinical protocols, regulatory standards, and infection control guidelines. Participate in interdisciplinary case conferences and clinical meetings as needed. Travel between patient homes throughout the designated service area. Only act within the scope of the individual’s authority to practice Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently. Perform nursing duties in DTC/Medical practice according to scope of license. Acting member of the IDT Designated Areas:Area 1: Bronx, Westchester, Upper ManhattanArea 2: Queens, Brooklyn, Lower ManhattanArea 3: Queens, Long Island Schedule: 8:30 AM – 5:30 PM Weekly Hours: 40 Qualifications Education: Degree from an NLN-accredited nursing school (RN, BSN required or at least five (5+) years of experience in health care setting). Experience Minimum of three (3+) years of clinical nursing experience, including 1-2 years in home care or a similar healthcare setting, preferred. Skills: Strong clinical assessment and critical thinking abilities, excellent communication skills, and the capacity to manage multiple participant cases effectively. Minimum of one (1) year of experience working with a frail or elderly population or, if the individual has less than one (1) year of experience but meets all other requirements, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring. Licensure: Current active and unrestricted license as a Registered Nurse in New York State. Additional Requirements Available for Weekends / nights/on-call coverage as needed. Ability to travel to participants’ homes and work in varying environmental conditions. Proficiency with computers and electronic health record (EHR) systems. Experience with long-term care or managed care systems beneficial. Vehicle and current New York State driver's license required preferred. Be legally authorized (for example, currently licensed, registered, or certified if applicable) to practice in the State in which the healthcare professional will perform the function. Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact. Physical Requirements Individuals must be able to sustain certain physical requirements essential to the job. This includes, but is not limited to: Standing – Duration of up to 6 hours a day. Sitting/Stationary Positions – Sedentary position in duration of up to 6-8 hours a day for consecutive hours/periods. Lifting/Push/Pull—Up to 50 pounds of equipment, baggage, supplies, and the ability to lift patients safely and using OSHA guidelines, etc. Bending/Squatting – Must be able to safely bend or squat to care for patients, use medical supplies, etc. Stairs/Steps/Walking/Climbing – Must be able to safely maneuver stairs, climb up/down, and walk to access work areas. The position requires the individual to be able to travel and walk between sites
