Role Description We are seeking a Registered Nurse Case Manager for an on-site contract position located in Bakersfield, CA. The primary responsibilities include coordinating patient care, creating and managing individualized care plans, ensuring effective discharge planning, conducting utilization management, and overseeing case management for patients. This role closely collaborates with medical professionals, patients, and their families to ensure high-quality healthcare outcomes. Qualifications Expertise in Case Management, including developing and implementing care plans Strong skills and experience in Discharge Planning to ensure optimal continuity of care Proficiency in Utilization Management to promote efficient use of healthcare resources Active RN license with a robust background in Nursing and medical best practices Exceptional communication and organizational abilities Familiarity with healthcare regulations and compliance standards Bachelor of Science in Nursing (BSN), or equivalent, is highly preferred
