Job Description
Position Summary Provides services to members in accordance with the State Nurse Practice Act, Accepted professional standards of practice, AFC service regulations, and Agency policy.Provide nursing services, utilize a comprehensive base of nursing theory and nursing process, and communicate/document observations and assessments.Qualifications Must be fully licensed by the Massachusetts Board of Registration in Nursing. Must have at least two years of recent experience in the direct care of elders or adults with disabilities Strong communication and interpersonal skills Compassionate, flexible, and team player Knowledge of basic computer skills Satisfactory status on background check Bilingual in Vietnamese preferred. Conduct required home visits (non-physical role).Essential Functions and responsibilities Completes and coordinates all applicable clinical assessments and clinical evaluations Develops AFC service plan of care Selects, supports, trains, and supervises caregivers in conjunction with the care manager Reports changes in the member's health status Reviews caregiver care logs Plans and implements discharges and transition from the AFC service Ensures implementation of the AFC service plan of care Coordinates the delivery of AFC service with any other health services or supportive services the member is receiving Conducts on-site visits with each member at the qualified home setting as follows: Completes a nursing progress note for each on-site visit or encounter and upon a significant change Educates the member about hygiene and health concerns Develops, in conjunction with the care manager and caregiver, and the member or responsible party, an emergency backup and personal care contingency plan for each member receiving AFC service that includes an alternative plan for the member if the caregiver is temporarily unable to provide care Participates in all mandatory education and training as required by the state Board of licensure and Agency policy Participates in team meetings; Shares areas of expertise and initiates interdisciplinary referrals Initiates actions to improve the quality of member care Initiates referrals to the community resources as needed to meet members' needs Maintains confidentiality in all aspects of the job Performs other duties and responsibilities as deemed necessary Job Types:Part-time or Full-time Expected hours:20 - 40 per week Benefits for full time employees:Dental insurance Health insurance Paid time off Retirement plan Work Location:On the road Job Types:Full-time, Part-time, Per diem Pay:$50.00 - $52.00 per hour Work Location:On the road.
