Position Overview We are seeking a compassionate and skilled Home Health Registered Nurse (RN) to provide high-quality, patient-centered care in the comfort of patients' homes.The ideal candidate is clinically strong, independent, and committed to improving patient outcomes through personalized care and education.Key Responsibilities Perform intake for admission, discharge, transfer, comprehensive patient assessments, including physical, emotional, and environmental needs Develop, implement, and update individualized care plans Administer medications, treatments, and wound care as prescribed Monitor patient progress and document clinical findings accurately Educate patients and families on disease management, medications, and self-care Collaborate with physicians, therapists, and interdisciplinary care teams Ensure compliance with all healthcare regulations and agency policies Respond to changes in patient condition and provide appropriate interventions Qualifications Active Registered Nurse (RN) license in Alaska Minimum of1 year of nursing experience is preferred(home health experience preferred) Strong clinical assessment and critical thinking skills Excellent communication and interpersonal abilities Ability to work independently and manage time effectively Valid driver's license and reliable transportation Current CPR/BLS certification Preferred Qualifications Experience with electronic health records (EHR) Knowledge of Medicare/Medicaid guidelines for home health Wound care certification or experience Benefits Competitive pay and mileage reimbursement Flexible scheduling Additional Requirements Successful completion of a background check is required prior to employment Why Join Us?
