Generations Home Health is looking for an RN for our Athens County/Meigs CountyTerritory. Flexible schedule!! Job Overview We are seeking a dedicated and compassionate Registered Nurse to join our home health team. In this vital role, you will provide personalized nursing care directly in patients’ homes, ensuring they receive the highest quality treatment and support. Your expertise will help promote health, independence, and well-being for individuals across diverse community settings. This flexible position offers the opportunity to make a meaningful difference while working independently and collaboratively with interdisciplinary teams. Responsibilities Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories to develop tailored care plans. Administer medications, including injections and IV infusions, following strict aseptic techniques and safety protocols. Perform wound care procedures such as suturing, ostomy management, and dressing changes with precision and compassion. Manage complex conditions like chronic illnesses, acute pain, infectious diseases, and palliative needs in a home environment. Coordinate with physicians and other healthcare providers to ensure continuity of care, discharge planning, and appropriate documentation in electronic health record (EHR) systems like Epic or Cerner. Educate patients and their families on disease management, medication administration, nutrition, and safety measures to foster independence. Maintain compliance with HIPAA regulations and adhere to all infection control standards while delivering patient-centered care. Skills Proven experience in diverse settings such as home care, hospital medicine, critical care units (ICU), or long-term care facilities. Strong knowledge of medical terminology, physiology, diagnostic evaluation, ICD coding (ICD-9/10), and medication administration protocols. Skilled in performing physical examinations, venipuncture (phlebotomy), sterile processing, and aseptic techniques. Familiarity with EMR/EHR systems including Epic, Cerner, Athenahealth or eClinicalWorks for accurate medical documentation. Ability to work independently while collaborating effectively with interdisciplinary teams across various specialties such as geriatrics, behavioral health, pediatrics, or hospice & palliative medicine. Excellent patient assessment skills combined with critical thinking to triage urgent needs and prioritize interventions. Experience working with individuals with disabilities or developmental delays is a plus; familiarity with Workers' Compensation law is beneficial. Knowledge of infection control practices and experience managing complex cases like dialysis patients or those requiring ventilator support is advantageous.
