Job Description
Interim HealthCare is seeking a compassionate and skilled Registered Nurse to join our Healthcare & Medical Services team providing home health care to patients in their homes and community settings. As a Home Health RN, you will deliver patient-centered nursing care, complete comprehensive assessments, and develop individualized care plans that reflect each patient’s unique needs, goals, and preferences. You will provide direct clinical care including medication administration, wound care, disease management education, chronic condition monitoring, and post-acute follow-up, while collaborating closely with physicians, therapists, aides, and family caregivers. In this role, you will conduct regular home visits, evaluate patient progress, update care plans, and document all care in accordance with clinical, regulatory, and Interim HealthCare standards. You will educate patients and families on medications, treatment plans, safety in the home, and strategies to maintain independence and quality of life. You will triage changes in condition, communicate promptly with providers, and coordinate services to prevent avoidable hospitalizations. Using your professional judgment, you will prioritize visits, manage a caseload, and ensure timely, accurate documentation in electronic health records.
