Registered Nurse (RN) – Home Care Per Diem

Overview Where The Heart Is Home Care is seeking a compassionate and dependable Registered Nurse (RN) to support our growing home care agency on a per diem basis. Responsibilities Conduct initial in-home client assessments Develop and update care plans Perform supervisory visits for caregivers Complete caregiver competency evaluations Provide clinical guidance and support regarding client concerns as needed Communicate client status updates with the care team as appropriate Maintain timely and accurate documentation Ensure compliance with NJ home care regulations and agency policies Qualifications: Active NJ RN license in good standing Minimum 1 year nursing experience preferred Home care experience preferred but not required Strong communication and documentation skills Reliable transportation and valid driver’s license Comfortable working independently within the community setting Position Details: Per diem / visit-based scheduling Flexible scheduling based on nurse availability No minimum hours guaranteed Opportunity for ongoing referrals and case assignments as the agency grows Initial assessments typically start at $100+ depending on travel and visit complexity Client visits primarily located throughout Union, Essex, and Middlesex Counties and surrounding areas We are looking for a responsive and reliable RN who is comfortable supporting clients, caregivers, and families in a home care setting.
