Registered Nurse (RN) – Home Health Location: Riverside, Moreno Valley, and Perris, CA Job Type: Full-Time, Part-Time, PRN Pay: $50 to $150 per visit, based on experience and visit type Job Description We are looking for a compassionate and dependable Registered Nurse (RN) to join our home health team serving patients throughout Riverside, Moreno Valley, and Perris . The RN will provide skilled nursing care to patients in their homes, complete assessments, follow the physician’s plan of care, and help ensure patients receive high-quality care in a comfortable home setting. Responsibilities Provide skilled nursing visits to patients in their homes Perform patient assessments and monitor changes in condition Follow the physician’s plan of care Provide wound care, medication teaching, disease management, and patient education Communicate with physicians, patients, families, caregivers, and the home health team Complete accurate and timely documentation Follow all agency policies and home health regulations Qualifications Active California RN license Home health experience preferred OASIS experience preferred Reliable transportation Valid driver’s license and auto insurance Strong communication and documentation skills Ability to work independently in the field Schedule Full-time, part-time, and PRN positions available Flexible scheduling Patient visits based on availability and service area Benefits Flexible schedule Supportive office and clinical team Growing home health agency Opportunity for steady visits Work Location Riverside, Moreno Valley, and Perris, CA Pay: $40.00 - $75.00 per hour Benefits 401(k) Dental insurance Flexible schedule Health insurance Work Location: In person
