Collaboration will be key, as you'll join forces with nurses, physicians, case managers, and fellow rehab experts to optimize patient outcomes. Educating patients and their families about therapeutic goals, discharge planning, and at-home exercises will also form a key part of your responsibilities. You'll meticulously document evaluations and progress in compliance with facility protocols and regulatory guidelines, while actively participating in interdisciplinary care rounds to ensure that our patients receive integrated and coordinated care.
