Schedule: Flexible hours Starting at 15-20 hours per week with potential for increased hours Job Description: We are seeking a compassionate and reliable Care Coordinator to provide in-home support services to individuals with developmental disabilities living in an independent setting in Aurora and Denver area. This role is ideal for someone who is passionate about making a difference in the lives of others and is looking for flexible, meaningful work during the week. As a care coordinator, you will play a vital role in supporting individuals with disabilities, mental health conditions, or other special needs in a variety of settings. Your primary responsibility will be to provide personalized care and support to help individuals lead fulfilling and independent lives. Responsibilities: Provide person-centered care and support tailored to the client’s unique needs Assist with daily living activities, including hygiene, meal preparation, and light housekeeping Support the client in maintaining routines and accessing community activities, if applicable Document daily services provided in compliance with DD waiver requirements Communicate effectively with agency staff and supervisors Provide transportation to individuals as needed. Implement and document individual goals and medical tracking electronically. Offer emotional support and companionship to individuals, helping to build trust and foster positive relationships. Manage challenging behaviors and develop strategies for behavior management. Maintain accurate records and communicate effectively with team members, families, and healthcare professionals. Requirements: Experience working with individuals with intellectual and/or developmental disabilities preferred Knowledge of behavioral management techniques and strategies for supporting individuals with complex needs.
