Job Description

You'll help shape decisions that elevate both patient outcomes and the future of nursing. Job Summary and Qualifications Performs direct patient care within the scope of the nurse practice act, as it applies to the outcomes and processes defined for Cardiovascular Service Line (CVSL) Programs, coordinates and implements care in the inpatient and outpatient setting for the cardiac patient as directed by program coordination requirements. An integral member of the CVSL Program team, the scope of the role includes acting as a liaison with physicians, patient/families and ancillary departments. Assessment of financial, medical and social resources of current and potential program patients in conjunction with the CVSL Program team. Coordination of information flow among the medical, nursing, financial and CVSL Program staff as appropriate. Development and implement a teaching plan with the patient and family, including lab values and diagnostics, medication overview and diagnosis related information. Assist the CVSL Program team in the coordination of patient charts, diagnostic studies and other applicable information. What qualifications you will need: Bachelors in Nursing is required from an accredited School of Nursing Basic Cardiac Life Support required 5 years’ experience with cardiac/vascular surgery and/or invasive cardiology is preferred Benefits Memorial Health University Medical Center, offers a total rewards package that supports the health, life, career and retirement of our colleagues.
