Job Description & Requirements RN Care Coordinator StartDate: 5/25/2026 Pay Rate: $1800.00 - $2700.00 POSITION SUMMARY – RN Care Coordinator POSITION DUTIES – Completing initial assessments, DC planning (including nursing homes, home health, DME, prior authorizations, etc) in an inpatient setting. MINIMUM REQUIRED QUALIFICATIONS – CA RN license; BLS; 3 years' nursing experience in an acute hospital setting; Prior care coordination experience in a CommonSpirit/Dignity/CHI hospital LENGTH OF ASSIGNMENT – 13 weeks SHIFT / HOURS PER WEEK – 8hr days SYSTEMS – Cerner START DATE – ASAP Facility Location Surrounded by mountain ranges, wildflowers and the Kern River, Bakersfield is the ideal destination for anyone seeking an abundance of recreational opportunities.
