RN Care Coordinator Per Diem

JOB DESCRIPTION AND POSITION REQUIREMENTS Scheduled Weekly Hours: TBD Day shift - weekday and weekend shifts JOB SUMMARY: Care Coordination is a collaborative process in which the Care Coordinator assesses, plans, implements, coordinates, and evaluates options and services to meet an individual's health needs. The Care Coordinator facilitates progression of care and monitors the patient's progress to ensure that the plan of care and services provided are patient focused, high quality, evidence based, appropriate to patient needs, efficient, and cost effective. Care Coordination improves access to services to improves outcomes and reduce over-utilization of resources. MINIMUM QUALIFICATIONS: Graduate of accredited school of Nursing, Baccalaureate degree preferred. Current RN licensure by the Louisiana State Board of Nursing 3-5 years of hospital adult acute care nursing experience preferred. Current BLS certification through any International Liaison Committee on Resuscitation (ILCOR) provided courses such as but not limited to: American Heart Association and American Red Cross. The ILCOR course must include a manikin check off. CCM, CMAC, or ACM preferred.
