Job Description
MLK Community Hospital, a private, nonprofit safety-net hospital serving South Los Angeles, is seeking a full-time Inpatient RN Care Manager to join our Healthcare & Medical Services team. In this role, you will combine your clinical expertise and care coordination skills to support patients throughout their inpatient stay and ensure safe, effective transitions of care. Working within a modern, innovative hospital environment, you will collaborate closely with physicians, bedside nurses, social workers, therapists, and other interdisciplinary partners to promote high-quality, culturally competent care for a historically underserved community. The RN Care Manager is responsible for comprehensive clinical and psychosocial assessments, development and implementation of individualized care plans, and ongoing evaluation of patient progress. You will coordinate all aspects of inpatient care management including discharge planning, utilization review, and facilitating appropriate post-acute services such as home health, rehabilitation, skilled nursing, and outpatient follow-up. Using your knowledge of community resources and insurance requirements, you will help patients and families navigate complex medical and social needs while advocating for timely, appropriate, and cost-effective care. On a daily basis, you will round with the interdisciplinary team, identify barriers to discharge, monitor length of stay, and proactively address clinical, social, and logistical issues that may impact patient outcomes. You will conduct patient and family education on diagnoses, treatment plans, medications, and self-management strategies, ensuring information is delivered in a culturally sensitive and understandable way. The RN Care Manager also engages in utilization management activities, including review of admission criteria, ongoing level-of-care review, and coordination with payers to support authorization and continued stay, while maintaining compliance with regulatory and accreditation standards. Success in this role requires strong clinical judgment, excellent communication, and the ability to manage a dynamic caseload in a fast-paced inpatient environment. You will use the electronic health record to document assessments, care plans, and progress notes, track key quality indicators, and contribute to performance improvement initiatives aimed at reducing readmissions, improving patient satisfaction, and enhancing overall care coordination.
