Job Description
HCA Healthcare is seeking an RN Case Manager Acute Rehab to join our Healthcare & Medical Services team, supporting high-quality care and safe transitions for patients in our acute rehabilitation setting. In this role, you will blend clinical nursing expertise with care coordination and discharge planning to ensure each patient receives individualized, patient-centered services that align with their rehab goals. Working closely with an interdisciplinary team of physicians, therapists, social workers, and nurses, you will assess patients’ clinical status, functional abilities, and psychosocial needs, then develop and manage comprehensive care plans that promote optimal recovery, appropriate length of stay, and successful discharge to the next level of care. You will collaborate with patients and families to provide education on treatment plans, rehabilitation expectations, medications, and community resources, while serving as an advocate to support informed decision-making and patient autonomy. The RN Case Manager Acute Rehab will coordinate services across the continuum, including arranging post-acute services such as home health, outpatient therapy, durable medical equipment, and follow-up appointments, as well as communicating effectively with payers and external providers to support authorization, utilization management, and coverage of services. You will monitor patient progress against established goals, identify barriers to discharge, escalate complex issues, and adjust plans as needed to maintain safety, quality, and regulatory compliance. Documentation of assessments, care plans, progress notes, and discharge summaries will be timely, accurate, and consistent with hospital policies, accreditation standards, and payer requirements. This position requires strong clinical judgment, excellent communication and organizational skills, and the ability to work in a fast-paced, team-based environment.
