Position Summary The RN Case Manager is responsible for planning and managing the nursing care of an assigned caseload of patients in home or long-term care settings. In collaboration with the interdisciplinary team, the RN Case Manager ensures that patient- and family-directed goals are achieved using a holistic, patient-centered approach. The role includes providing education, facilitating communication, and coordinating care across various sites. Flexibility in work location across care settings is required.
