The Registered Nurse Case Manager provides a wide range of nursing care to patients in the home care setting. The RN Case Manager renders professional nursing care by assessing, developing, implementing and evaluating home care needs. This position will serve Trumbull County. Looking for full time and part time candidates. Responsibilities Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients. Assess, plan, direct and evaluate each patient's care plan to ensure it meets the patient's needs. Manage emergency situations in a prompt, precise, and professional manner. Complete medication administration and treatment needs for all patients assigned. Provide nursing care in accordance with applicable regulations and company policies. Prepare and maintain accurate and complete clinical and progress notes on a timely basis Provides supervision and oversight to patient care. Utilize the Electronic Visit Verification system. Prepares physician orders as applicable. Participates in continued education programs, patient care conferences and staff meetings as scheduled. Performs nursing assessments (initial and ongoing).
