RN Case Manager - Full Time

Work Schedule: Monday-Friday Pay Rate: $43–$45 per hour BrightStar RN Benefits: 401K match (up to 4% match) after 6 months of employment Paid time off Medical Benefits (including Health/Dental/Vision) Mileage Reimbursement Free training and continued education Weekly pay w/ direct deposit Referral program Responsibilities of an RN Case Manager: No OASIS work! Provides care as defined within the scope of the state’s nurse practice act. Makes the initial evaluation visit and regularly reevaluates the client’s nursing needs. Prepares clinical and progress notes and summaries of care. Serve as backup for Critical Care Clients, In-School Staffing & PDN Assures proper maintenance of clinical records in compliance with local, state, and federal laws. Helps educate and coach care-giving staff and assists with new hire clinical training, as needed. RN Case Manager Requirements: Current unrestricted RN license in Minnesota Minimum two (2) years of clinical experience with one (1) year of experience in a home care agency, primary care clinic, or health facility, preferred. Critical Care Experience (1-2 years) preferred Active CPR Certification Negative TB skin test or QuantiFERON-TB Gold (QFT) blood test (completed within the last 90 days) Reliable car & Auto Insurance Valid Driver's License Clean background and Criminal Record Understands regulatory compliance with state and federal regulations.
