RN Case Manager

HCA Healthcare is seeking an RN Case Manager to coordinate patient care across the continuum in our Healthcare & Medical Services department. In this role, you will perform clinical and psychosocial assessments, develop and manage individualized care plans, and ensure safe, efficient transitions of care. You’ll collaborate closely with an interdisciplinary team, conduct utilization review, and educate patients and families on resources and follow-up care.
