Position:RN-Case Manager-Wailuku,HI Shift:08:00 AM to 04:30 PM Job Overview We are seeking a highly skilled and compassionate Registered Nurse (RN) Case Manager to join our healthcare team.In this vital role, you will coordinate patient care across various settings, ensuring seamless transitions from inpatient to outpatient services, post-acute care, and community resources.Your expertise in clinical assessment, discharge planning, and utilization management will support optimal patient outcomes and enhance overall healthcare delivery.The ideal candidate will possess extensive experience in critical care environments and a comprehensive understanding of complex medical conditions, including trauma, infectious diseases, and chronic illnesses.Duties Conduct thorough patient assessments, including vital signs, physiological evaluations, and medical history reviews to determine care needs.Develop individualized care plans in collaboration with multidisciplinary teams, ensuring alignment with patient goals and clinical guidelines.Coordinate discharge planning by arranging post-acute services, outpatient follow-up, and community resources to facilitate smooth patient transitions.Manage utilization review processes by evaluating medical necessity for procedures, admissions, and continued stays in accordance with healthcare policies.Monitor patient progress through diagnostic evaluations and ongoing assessments, adjusting care plans as necessary to optimize recovery.Maintain accurate medical records and documentation in compliance with HIPAA regulations and hospital standards.Collaborate with physicians, therapists, social workers, and other healthcare professionals to ensure comprehensive patient care.Educate patients and families about treatment options, medication management, and health maintenance strategies.Participate in quality improvement initiatives aimed at enhancing patient service delivery and clinical outcomes.Support hospital operations by contributing to case management meetings and reporting on patient progress.Skills Extensive critical care experience within ICU or Level II trauma centers; experience in Level I Trauma Centers is highly desirable.Proven expertise in hospital medicine, emergency medicine, infectious disease care, pediatrics (PICU), geriatrics, behavioral health, and medical-surgical units.Strong knowledge of physiology principles relevant to acute and chronic conditions.Proficiency in medical terminology, diagnostic evaluation techniques, and medical records management.Familiarity with managed care protocols, utilization management processes, discharge planning procedures, and outpatient clinic operations.Excellent patient assessment skills combined with the ability to interpret vital signs and clinical data accurately.Experience working within inpatient or outpatient settings with a focus on post-acute care coordination.Knowledge of HIPAA compliance standards for safeguarding patient information.Effective communication skills for engaging with diverse patient populations and multidisciplinary teams.Ability to navigate complex healthcare systems using strong organizational skills and attention to detail.This position offers an engaging opportunity for dedicated nursing professionals committed to delivering exceptional patient-centered care across multiple clinical environments.We welcome applicants who are eager to utilize their specialized skills in a dynamic healthcare setting dedicated to improving lives through expert case management.Mandatory:Hospital experience is required #Nurse2026 Job Types:Full-time, Contract, Temporary Pay:$2,000.00 - $2,566.00 per week Benefits:Dental insurance Health insurance Work Location:On the road.
