Hospice RN Case Manager Location: Coverage area spans Brookline/Brighton to Mansfield (Scheduler clusters visits by geographic area) Schedule Monday-Friday | 9:00 AM-5:00 PM No weekends or holidays Start Date: July 13 This assignment is to cover a 6-month maternity leave. The RN must be available to start a few weeks earlier if the incumbent nurse begins leave sooner. Assignment Length: 26 weeks to start Compensation Contract Rate: $57/hour Mileage Reimbursement: $0.725 per mile (reimbursed from first appointment to last appointment; does not include commute to/from home) Parking: Fully reimbursed About the Partner Organization Amergis Healthcare Staffing is partnered with a multicultural boutique hospice agency providing compassionate, patient-centered care in homes, nursing facilities, and assisted living communities. As a Hospice RN Case Manager, you'll play a vital role in supporting patients and families with comfort-focused, holistic care during the end-of-life journey. Key Responsibilities Conduct 5-6 home or facility visits per day Assess patient condition and monitor symptom progression Collaborate with physicians and interdisciplinary team members on individualized care plans Provide comfort-focused care tailored to each patient's needs Partner closely with caregivers to ensure continuity of care Administer medications and manage pain and symptom control Respond to acute changes in condition and escalate to physicians as needed
