Incredible Health is seeking a skilled and motivated Registered Nurse (RN) to deliver high-quality, evidence-based patient care while leveraging technology to enhance outcomes and streamline clinical workflows. As an RN within the Healthcare & Medical Services department, you will work in a fast-paced, dynamic environment supporting patients, providers, and care teams through both direct and technology-enabled care. You will use your clinical expertise alongside Incredible Health’s digital tools to improve care quality, efficiency, and coordination for the patients and facilities we serve. In this role, you will conduct comprehensive patient assessments, develop and implement individualized care plans, administer medications and treatments safely, and provide clear education to patients and families. You will document accurately and efficiently in Electronic Health Records (EHR), ensuring compliance with regulatory, safety, and quality standards. You will collaborate closely with physicians, advanced practice providers, therapists, social workers, and other interdisciplinary team members to coordinate care across the continuum, using data and digital platforms to support clinical decision-making and communication. You will triage patient needs both in person and through phone or virtual channels, escalating care appropriately and leveraging telehealth tools where applicable. You will participate in quality improvement initiatives, infection prevention practices, and safety programs, identifying opportunities to optimize processes and outcomes.
