Job Description
KNOWLEDGE BASE: Current Registered Nurse (RN) or Licensed Vocational Nurse (LVN/LPN) license in good standing in the State of Texas Practices nursing in accordance with the Texas Nurse Practice Act and the rules of the Texas Board of Nursing Strong assessment, clinical judgment, and documentation skills Effective verbal and written communication skills Ability to organize workflow, manage multiple priorities, and respond calmly to emergencies Genuine interest in caring for elderly and disabled individuals Maintain regular, predictable attendance and report to assigned shifts on time in accordance with facility attendance and call-out procedures Perform duties requiring standing, walking, bending, lifting, repositioning residents, and responding rapidly to emergencies Work assigned shifts, including weekends, holidays, and overtime as required by resident care needs and staffing levels Make timely, independent nursing decisions in high-stress or time-sensitive situations Report to work mentally and physically fit for duty and free from impairment that could affect safe nursing practice Immediately notify supervision of any condition that may impair the ability to practice safely Provide nursing care in accordance with physician orders, individualized care plans, facility policies, and applicable laws and regulations Exercise independent nursing judgment and take appropriate action to protect resident safety Properly administer medications and treatments and monitor residents for therapeutic and adverse effects Accurately and promptly transcribe, implement, and follow physician orders Seek clarification of incomplete, conflicting, or questionable orders prior to implementation and document actions taken Complete nursing assessments as required and implement individualized care plans Perform treatments and procedures within scope of practice Conduct routine rounds to monitor resident condition, care delivery, and safety Participate in shift report and communicate pertinent resident information to the interdisciplinary team Notify supervisor and follow facility policy when an assignment, order, or condition is unsafe, unclear, or outside the nurse's scope of practice Maintain timely, accurate, and complete documentation of resident care, assessments, medication administration records (MARs), and incident reports Utilize electronic health record systems and medication management software in accordance with facility policy Ensure documentation meets facility, CMS, HHSC, and legal standards Maintain confidentiality of resident and staff information in accordance with HIPAA and facility policy Provide direction and supervision to Nurse Assistants and Medication Aides to ensure care is delivered in accordance with care plans and facility policy Delegate and supervise nursing tasks in accordance with the Texas Nurse Practice Act, Texas Board of Nursing rules, and facility policy Remain accountable for nursing care delegated to unlicensed personnel and Medication Aides Adjust assignments and respond to staffing shortages by arranging replacement staff as necessary Participate in new employee orientation, mentoring, and performance feedback Communicate significant changes in resident condition, incidents, or care plan updates to physicians, families, and responsible parties Collaborate with the interdisciplinary team to coordinate admissions, discharges, and transfers Demonstrate professionalism when interacting with residents, families, visitors, and staff, including during challenging or emotionally charged situations Maintain a safe working environment and comply with infection control, body mechanics, and safety procedures Demonstrate knowledge of emergency procedures and participate in drills and actual emergency responses Immediately report suspected abuse, neglect, exploitation, misappropriation, unsafe conditions, or unusual occurrences in accordance with federal, state, and facility policies Maintain appropriate professional boundaries with residents, families, and coworkers Recognize and respect resident rights, dignity, cultural backgrounds, and privacy Practice nursing in full compliance with the Texas Nurse Practice Act and all rules, standards, and ethical obligations of the Texas Board of Nursing Maintain an active nursing license in good standing and immediately notify the facility of any restriction, investigation, suspension, or other change in licensure status Delegate and supervise nursing tasks appropriately and remain accountable for all nursing care delegated to unlicensed personnel and Medication Aides Seek clarification of incomplete, conflicting, or questionable physician or provider orders prior to implementation and document actions Report to work mentally and physically fit for duty and free from impairment that could affect safe nursing practice Immediately report suspected abuse, neglect, exploitation, misappropriation of property, unsafe conditions, or unusual occurrences as required
