Develops individualized plans of care in collaboration with the patient, family, and healthcare providers, ensuring continuity and documentation. Implements interventions from the plan of care safely, timely, and appropriately, documenting actions taken. Evaluates patient progress towards outcomes, revising diagnoses, outcomes, and plans of care as needed. Administers medications and treatments as prescribed, ensuring patient safety and adherence to protocols. Communicates effectively with patients, families, physicians, and staff, maintaining professional relationships and promoting a positive environment. Educates patients and families on health conditions, procedures, and care plans, ensuring understanding and compliance. Participates in quality improvement activities to enhance patient care and nursing practices. Serves as a patient advocate, ensuring patient rights and needs are respected and addressed. Collects and documents patient health data systematically and continuously. Analyzes assessment data to determine and validate diagnoses with the patient, family, and healthcare providers. Knowledge, Skills, And Abilities Administers all types of medications correctly per age group for all populations served according to policy Demonstrates proficient technical skills and operational knowledge of all equipment used on the unit, including equipment used to stabilize and monitor patients (i.e., IV insertion, central lines, pulse ox, NG tubes, Foley, chest tubes, hemodynamics monitoring, etc.) Proficient in the use of electronic medical records and automated dispensing medication systems for efficient medication administration and patient data retrieval Performs daily rotational task assignments as directed Supervises Nursing Assistants in the process of basic nursing functions, ensuring adherence to the patient’s cultural needs and demonstrating standard procedures for safe patient ambulation, transfers, and repositioning Serves as a preceptor as necessary, guiding and instructing less experienced staff in the nuances of patient care Engages in effective communication and collaboration with the healthcare team, including gathering and reassessing patient data, initiating and implementing physician orders, and contributing to care plan development Demonstrates a comprehensive understanding of emergency codes, infection control protocols, life safety, fire plans, and age-specific safety concerns, and supervises nursing assistant staff during these events Ensures the provision of necessary and appropriate education to patients and their families from admission through discharge, covering medical and nursing plans of care, medications, tests and procedures, disease processes, and discharge instructions. Documents education provided, including patient responses and outcomes Initiates referrals to healthcare providers as necessary Fluent in English with excellent reading, verbal, and written communication skills for effective interaction with patients, families, and the healthcare team Education Associate's of Nursing [Required] Bachelor's of Nursing [Preferred] Work Experience
