Position Summary The Nurse Manager is responsible for the directing, planning, evaluation and coordinating of patient care on our 16 bed rehab unit. Support Culture of Excellence through active role with assigned neighborhood initiatives. Essential Job Functions: Provides supervisory oversight on the nursing unit to ensure quality of care through daily unit rounds, audit completion, care card review and monitoring compliance with department systems. Understanding of and commitment to continuous quality improvement. Responsible for one nursing unit 24 hours per day. Completes day shift performance appraisals. Supervises and participates in the development, implementation, and evaluation of the nursing care for Members within the unit. Assists with medical examinations, confers with doctors in regard to medical problems; sees that the medical plan of care is carried out. Supervises and participates in the maintenance of records, reporting, and interpretation of the Members' outcome of care. Provides on unit training to nursing staff as needed. Collaborates with appropriate members of the interdisciplinary team to establish a total plan of care for each Member. Participates in periodic evaluation of staff performance and in disciplinary and evaluation conferences. Participates in defining, maintaining and interpreting standards of quality nursing practice by making rounds to evaluate quality of care provided. Provides guidance and teaching to nursing staff in cooperation with Staff Education Coordinator to assure orientation and on-going learning of the nursing staff. Conducts regular meetings with the staff to promote communication and continual growth. Establishes good relationships with member families and provides on going communication. Demonstrates excellent customer service skills. Ensures that all Members are receiving the necessary care and services to enhance and promote quality of life. Documents all required Member care information accurately and timely in My Unity. Notifies Member families with changes to Members plan of care. Responsible for implementation, ongoing monitoring and audit completion at the Unit Level for the Survey Plan of Correction. Daily communication huddles with all shifts to ensure timely communication of information and staff feedback. Accountable to ensure all neighborhood daily documentation is completed. Responsible for collaborating with skin care team for treatment of pressure ulcers and weekly pressure ulcer log. Maybe responsible for providing on call unit coverage on weekends, evenings and nights. May attend meeting and workshops to improve job skills and/or working relationships. Responsible for MDS 3.0 completion on unit and Member care planning. Complies with HIPPA regulations. Performs other duties as required by Director of Health Services or Administration from time-to-time. Participates in Monthly quality Assurance and recruitment meetings. Expectations of the Position: Dependable, report to work on-time for every schedule shift (ready to work at beginning - start of shift) and work full schedule shifts unless physically unable or pre-approved Paid Time Off (PTO) which includes personal and vacation. Willing and able to work overtime and weekends as required by the position and/or approved by management. Have reliable transportation to and from work.
