RN Visit Nurse

Interim HealthCare is seeking a compassionate RN Visit Nurse to join our Healthcare & Medical Services team, providing patient-centered care in the home setting. As a key member of our home health team, you will deliver skilled nursing visits to patients where they live, focusing on safety, comfort, and optimal health outcomes. You will conduct comprehensive nursing assessments, develop and update individualized care plans, administer medications and treatments as ordered, and monitor changes in patient condition, communicating promptly with physicians, caregivers, and the broader care team. Using your clinical judgment, you will educate patients and families on disease management, medications, wound care, and preventive health practices, empowering them to participate actively in their care. You will document all care accurately and timely in the electronic medical record, ensuring regulatory compliance and continuity of care. In collaboration with therapists, aides, and other clinicians, you will coordinate services to support safe transitions from hospital or facility to home and help prevent avoidable readmissions.
