Job Description We are looking for a Rural Health Transformation Project Manager to support Indiana's Rural Health Transformation Program (RHTP) Initiative 6: Growing Access to Hospital Post Discharge Medications. This position will be Full-Time and Hybrid , working onsite three days per week at the Indiana Department of Health (IDOH) office in downtown Indianapolis, with the option to work remotely two days per week. Only candidates without a need for sponsorship now or in the future will be considered. Candidates must currently be a commutable distance to downtown Indianapolis.
